This notice reflects the core requirements of Section 1557 of the Affordable Care Act and its implementing regulations. The federally required set of translated taglines, and the grievance-procedure and civil rights coordinator requirements that apply to practices above a certain staff size, should be confirmed by a qualified healthcare compliance professional or attorney before this notice is relied upon as your practice's final, legally binding version.

Serenity By A Dr. Sareen complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity). We do not exclude people or treat them differently because of any of these characteristics.

Services We Provide

  • Serenity By A Dr. Sareen provides, free of charge, aids and services to people with disabilities so they can communicate effectively with us, including:
  • Qualified sign language interpreters
  • Written information in other formats, such as large print, audio, or accessible electronic formats
  • Free language services to people whose primary language is not English, including qualified interpreters and information written in other languages

How to Request Assistance

If you need any of these services, please call the office nearest you and ask. There is never a charge to you for interpretation or for accessible formats.

How to File a Grievance

If you believe that Serenity By A Dr. Sareen has failed to provide these services, or has discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you may file a grievance with our office by phone at (561) 879-4006 or by email at drsareen@serenitybyadrsareen.com. You may file a grievance in person, by mail, by fax, or by email, and we can help you file one if you need assistance. Filing a grievance will never affect the care you receive from us.
